Root Cause Analysis is looking for the most basic cause(s) of a problem that we have control over to fix, so the problem will not recur or will have a much lower likelihood to recur.


When conducting a root cause analysis we base it on facts and data to bring hidden issues to the surface so that we can see them and deal with them.


When in a kaizen event (seen in an earlier post) we often do root cause analysis, then put countermeasures in place such as:


Standards operating procedures (SOPs)

Training

Maintenance practices

Supplier quality/reliability

Workplace organization

Production planning/scheduling

Performance measures

Clear expectations

Accountability


There are several tools used for root cause analysis but next week I will share one of my favorites.
